Payment Options

• To receive the 35% cash discount, you may pay by cash or check at the time of treatment. You may also choose to be billed later for the full treatment amount.
• If you would like to pay with a card, I can send a PayPal invoice to your email.

Insurance

Currently, I am a preffered provider with:
• Premera Blue Cross
• Lifewise of Washington
• First Choice Health Networks

I will submit claims for these companies.

If you have another insurance and have out of network benefits, acupuncture and massage maybe covered. You will need to pay at the time of service and then I will give you all the information you need to self-submit a claim. Please contact your insurance company for more information.

Personal Injury Claims

If you have been in a motor vehicle accident and have a personal injury claim (PIP), insurance companies will generally cover acupuncture as long as your claim is approved. I will happily submit PIP claims for you.

Work-Related Claims

• Washington State Labor and Industries does not cover acupuncture, massage therapy is covered.
• If you have a Federal Worker’s Compensation claim, please ask your claim manager if acupuncture is covered by your claim. A perscription from a medical doctor is required.
